CISS validated as a useful clinical tool for older adults with binocular vision issues

The Convergence Insufficiency Symptom Survey (CISS) is a valid and reliable tool for assessing convergence insufficiency (CI) symptoms in presbyopic adults, according to a study. However, its multidimensional nature suggests that it should be complemented with objective clinical tests for a more comprehensive evaluation.

Researchers assessed 100 adult patients with normal binocular vision (n = 50) or CI (n = 50). The Persian-translated CISS was administered twice over a 7–14-day period, and participants underwent vision tests, including refraction, cover test, near point of convergence, and fusional range measurements.

Significantly higher CISS scores were found in the CI group (19.24 ± 5.33) compared to the normal group (13.26 ± 3.53, P < 0.001). A cutoff score of 15 effectively distinguished CI, with a sensitivity of 72% and specificity of 69%. The CISS demonstrated strong internal consistency and excellent test-retest reliability (ICC = 0.91). Rasch analysis confirmed good model fit and measurement precision, though multidimensionality was observed, with headaches reported more frequently in the CI group.
